After completing the survey, you’ll be asked if you’d like to enter the sweepstakes. Select “Yes” to join the $75 gift card drawing. - Provide Contact Info:

Five Guys Survey Rules
- Eligibility: Must be 18 years or older and a legal resident of Canada.
- One Entry per Day: You can only enter once per day, per receipt.
- Survey Completion Deadline: Complete the survey within 30 days of your purchase.
- Employees Not Eligible: Five Guys employees cannot participate.
- Taxes: Winners are responsible for taxes on the prize.
Monthly Prize Draws & Winner Notification
- 120 Winners Annually: Ten winners will be randomly selected each month.
- Drawing Dates: Winners are selected on or around the 10th of each month.
- Winner Notification: Winners will be contacted via email and must confirm their mailing address within 48 hours.
